What’s the difference between “hear” and “listen”?

Both words seem very similar to me. Sometimes I’m not sure which one to use. Is there a simple way to remember the difference?

Hear: A physical sense; sound just enters your ears.
Listen: A mental action; you pay attention to the meaning.

“Hear” happens naturally. “Listen” means you are paying attention. For example:
    I heard a noise.
    I listened to the music.
